Home Sweet Home Senior Living Advisor is dedicated to serve each client based on their individual needs. We consider their mental, spiritual, social and physical well being as well as personal finances in order to match them with the very best community to suit their lifestyle.
Free Initial Senior Needs Assessment – We start by asking about individual preferences, health care needs and finances in order to narrow the search to find the perfect fit home. After determining next steps, Home Sweet Home Senior Living Advisor charges a small search fee which will be refunded to the client when a successful move it made.
Education and Recommendations – We will do the research for you! We save you precious time by guiding families through the most appropriate home options that meet their needs.
Tours and Transition– Our advisor will coordinate tours, facilitate communications and personally attend to all the details. As advocates, we act as a liaison with the facilities to ensure a smooth transition. We can help coordinate down-sizing, a physical move and other move related services.
Follow Up – Finally we provide follow up to further demonstrate our commitment to excellence to ensure the resident is safe, comfortable and happy in their new "Home Sweet Home"
Unlike many of the larger senior living placement companies, Home Sweet Home Senior Living Advisor is different as we are a local, women-owned business who has built real relationships in the community. We will provide personalized unbiased advice and guidance for a variety of community resources.
The personal Senior Needs Assessment will give us insight into the care needs of the client. The amount of assistance needed with activities of daily living is often an indicator of the type of housing and care that will be needed now and in the future. Each facility is unique so our advisor will work to ensure all of your specific needs are met.
Our Advisor will work to find you the best living option no matter what your financial situation, whether you are seeing luxury living or in need of quality services on a budget. Housing rent and service charges vary greatly but there are apartments available to fit your lifestyle. There are many programs and options including VA Aid & Attendance, Affordable Housing, Medicare and Medicaid Programs and companion suites that may work within your budget.
If you are not ready for a move into a senior living community, We can develop an individualized care plan for you to help you stay at home and navigate the complexities of senior care options as your personal advocate.
We act as a support system for both the client and the family. We discuss difficult topics and can help navigate complex issues.We also provide expert coordinated care. We develop short-and-long-term care plans. We advocate for our client’s needs.
The placement services into Independent, Assisted or Memory Care Communities are free to the client and our advisor is paid through a referral fee from the community only after a successful move.
The client can choose to pay a refundable* search fee or pay an hourly fee for advice and guidance.
*Refundable after a successful move into a recommended community
Care Management Services are privately paid by the client or their loved one and the rate is dependent on their individual needs. Contact me for more information
